Achilles tendinopathy treatment in South Woodford: Insertional Achilles Pain and Choosing Exercises
Pain at the heel attachment of the Achilles can need a different starting exercise range from pain higher in the tendon. At South Woodford, we assess the location and help choose a plan that suits the presentation. A routine copied from a runner with a different tendon problem can aggravate symptoms rather than help you progress.
Understanding Achilles tendon pain
Achilles tendinopathy can involve the middle of the tendon or its attachment to the heel. Pain or stiffness may interfere with walking, running and calf raises. An insertional problem may need different exercise choices from mid-portion pain; a programme found online is not automatically right for both.
How we assess Achilles tendon pain at our South Woodford clinic
We assess the symptom location, recent loading changes, calf function and whether the history raises concern about a rupture. We also ask about previous treatment and relevant medical factors. Imaging is reserved for a question it can usefully answer, such as suspected tearing or diagnostic uncertainty.
A practical plan for this problem
Tell us whether the pain is directly at the heel and which footwear or movements affect it. We review calf function and choose a loading range that is tolerable before adding resistance or other demands. If symptoms remain persistent, we can discuss a further diagnostic review and shockwave through the network. A sudden loss of push-off or a snap needs urgent medical assessment.
Osteopathy and rehabilitation: what treatment may involve
Rehabilitation generally centres on progressively rebuilding calf and tendon capacity. Your starting exercise range, resistance and recovery time are selected according to the presentation. Osteopathic care may address associated movement restrictions, but massage alone does not build the tendon capacity needed for a return to demanding activity.
Does shockwave have a role?
Radial or focused shockwave may be discussed for selected persistent Achilles tendinopathy alongside a rehabilitation plan. NICE notes limited and inconsistent evidence and requires special arrangements for governance, consent and audit or research. We explain uncertainty and monitor progress rather than promising that a fixed number of sessions will resolve it.
Assessment and rehabilitation can start at South Woodford. Where shockwave is appropriate, we can direct you to radial treatment at Loughton or focused treatment at Epping; diagnostic ultrasound is also available at Epping.
When to seek medical help
A sudden snap or pop with loss of push-off strength can indicate a rupture. Seek urgent medical assessment rather than booking shockwave. A swollen painful calf, particularly with redness or warmth, also needs urgent advice; breathlessness or chest pain requires emergency help.
Questions patients often ask
Should I lower my heel below a step to stretch it?
Not automatically, especially with insertional symptoms. The movement range should be selected for your presentation and adjusted to the response rather than imposed because it is a common exercise.
How will we know whether the plan is helping?
We agree a useful functional goal and review changes in symptoms and activity. The number of appointments depends on the findings and response. If progress is poor, the plan should be reassessed and another opinion or investigation considered where appropriate.
