Hip pain treatment in South Woodford: Hip Pain During Longer Walks and a Sustainable Plan

Outer-hip pain can make a longer walk difficult even when shorter trips are manageable. At South Woodford, we assess the pain location and how symptoms respond to activity. We can then help find a walking and strengthening plan that starts from your current tolerance rather than repeatedly asks the hip to do more than it can manage.

Understanding outer hip pain

Greater trochanteric pain syndrome, or GTPS, describes pain around the outside of the hip. It may involve the gluteal tendons and nearby tissues. It is not the same as every kind of hip pain: groin symptoms, back-related pain and joint conditions need their own assessment.

How we assess outer hip pain at our South Woodford clinic

We ask about side-lying, stairs, walking and standing on one leg, then assess relevant hip movement and muscle function. Back symptoms and the location of the pain help guide the differential diagnosis. Ultrasound may be useful for a specific tendon question where the findings would alter treatment.

A practical plan for this problem

Describe whether hills, pace or time on your feet changes symptoms, and whether side-lying is uncomfortable. We may select a smaller repeatable walking dose and suitable hip exercises, progressing them as function improves. If the findings suggest refractory GTPS, shockwave at another clinic can be discussed carefully. Groin pain, significant restriction or back-related symptoms may need another pathway.

Osteopathy and rehabilitation: what treatment may involve

An appropriate plan may include reducing irritating compression, developing hip strength and gradually increasing walking or exercise. Hands-on treatment may support comfort, but the plan also needs to rebuild activity tolerance. If progress is poor, we revisit the diagnosis rather than simply repeating the same treatment.

Does shockwave have a role?

Shockwave can be discussed for selected refractory GTPS. NICE says evidence on safety and efficacy is limited and requires special arrangements for governance, consent and audit or research. Pain during or after treatment and possible worsening should be discussed, alongside rehabilitation and other management options.

Assessment and rehabilitation can start at South Woodford. Where shockwave is appropriate, we can direct you to radial treatment at Loughton or focused treatment at Epping; diagnostic ultrasound is also available at Epping.

When to seek medical help

Seek urgent medical advice for severe hip pain following a fall, inability to bear weight, or a hot painful joint with fever. Persistent unexplained night pain, systemic symptoms or progressive weakness should be assessed medically.

Questions patients often ask

Can I begin treatment before deciding about shockwave?

Yes. Local assessment and rehabilitation are the starting point. Shockwave is an additional option for selected presentations after discussion, not a requirement for every hip complaint.

How will we know whether the plan is helping?

We agree a useful functional goal and review changes in symptoms and activity. The number of appointments depends on the findings and response. If progress is poor, the plan should be reassessed and another opinion or investigation considered where appropriate.
