Hip pain treatment in Loughton: Hip Pain on Stairs and Single-leg Loading
Pain on stairs or when standing on one leg can point towards an outer-hip problem, but it is not a diagnosis on its own. At Loughton, we assess the symptom location and relevant movement to help plan care. The practical objective is to improve the tasks you use every day while avoiding unnecessary repeated aggravation.
Understanding outer hip pain
Greater trochanteric pain syndrome, or GTPS, describes pain around the outside of the hip. It may involve the gluteal tendons and nearby tissues. It is not the same as every kind of hip pain: groin symptoms, back-related pain and joint conditions need their own assessment.
How we assess outer hip pain at our Loughton clinic
We ask about side-lying, stairs, walking and standing on one leg, then assess relevant hip movement and muscle function. Back symptoms and the location of the pain help guide the differential diagnosis. Ultrasound may be useful for a specific tendon question where the findings would alter treatment.
A practical plan for this problem
Tell us whether symptoms are outside the hip, in the groin or associated with back pain. We may assess tolerable stepping and hip strength to establish a starting level. Rehabilitation can then develop the capacity required for stairs and longer walks. For a suitable refractory tendon problem, radial shockwave may be discussed alongside exercise, with careful explanation of the evidence limitations.
Osteopathy and rehabilitation: what treatment may involve
An appropriate plan may include reducing irritating compression, developing hip strength and gradually increasing walking or exercise. Hands-on treatment may support comfort, but the plan also needs to rebuild activity tolerance. If progress is poor, we revisit the diagnosis rather than simply repeating the same treatment.
Does shockwave have a role?
Shockwave can be discussed for selected refractory GTPS. NICE says evidence on safety and efficacy is limited and requires special arrangements for governance, consent and audit or research. Pain during or after treatment and possible worsening should be discussed, alongside rehabilitation and other management options.
Radial shockwave can be incorporated into an appropriate osteopathy treatment plan at Loughton. If focused shockwave or diagnostic ultrasound is indicated, we can discuss the Epping service with you.
When to seek medical help
Seek urgent medical advice for severe hip pain following a fall, inability to bear weight, or a hot painful joint with fever. Persistent unexplained night pain, systemic symptoms or progressive weakness should be assessed medically.
Questions patients often ask
Should I stretch the sore outer hip every day?
Stretching is not automatically the right answer to tendon pain. Some positions may be irritating. We choose exercises and activity changes according to the assessment and your response.
How will we know whether the plan is helping?
We agree a useful functional goal and review changes in symptoms and activity. The number of appointments depends on the findings and response. If progress is poor, the plan should be reassessed and another opinion or investigation considered where appropriate.
