Shoulder pain treatment in South Woodford: Shoulder Pain at Night and How to Plan Care

A shoulder that aches at night can affect energy and concentration the next day. At South Woodford, we assess the pattern and whether it fits a tendon, joint or referred pain presentation. Night symptoms deserve attention, but they do not automatically mean a tear or prove that you need a scan.

Understanding shoulder pain

Shoulder pain when reaching, lifting or lying on one side can have several causes. A tendon-related presentation differs from a frozen shoulder, joint arthritis or pain referred from the neck. A painful movement test alone is not enough to say that a tendon is torn.

How we assess shoulder pain at our South Woodford clinic

Assessment considers your active movement, how the shoulder moves with assistance, strength and the effect of neck movement where relevant. We ask about injury, sleep disruption and weakness. Diagnostic ultrasound may help answer a specific soft-tissue question if the result would change management.

A practical plan for this problem

Describe whether pain is linked to lying on one side, reaching or an injury, and whether you have weakness or marked stiffness. A plan may include a more comfortable sleep setup and suitable shoulder loading during the day. If symptoms are persistent or findings suggest another condition, we discuss GP review or imaging. Tendon-specific advanced services can be accessed through the network where appropriate.

Osteopathy and rehabilitation: what treatment may involve

For suitable musculoskeletal shoulder pain, we may combine osteopathic treatment with graded shoulder exercises and advice about lifting or reaching. Exercise range and resistance are adjusted to your response. Persistent major restriction, substantial weakness or a traumatic onset may require medical or specialist assessment.

Does shockwave have a role?

Shockwave may be discussed for selected persistent tendon problems after diagnosis and rehabilitation review. It is not appropriate for every shoulder problem. NICE recommends shockwave for calcific shoulder tendinopathy only in research, so we do not market it here as an established routine solution for calcium deposits.

Assessment and rehabilitation can start at South Woodford. Where shockwave is appropriate, we can direct you to radial treatment at Loughton or focused treatment at Epping; diagnostic ultrasound is also available at Epping.

When to seek medical help

Get urgent medical advice after an injury if you cannot move your arm, the shoulder changes shape or swelling is severe. A hot joint with fever needs urgent review. Shoulder or arm pain with chest pain, breathlessness or sweating warrants emergency help.

Questions patients often ask

Is resting the shoulder completely the best approach?

Not for every presentation. Suitable movement often remains part of care, but the range and load should fit the diagnosis. Severe injury symptoms need medical assessment rather than a standard exercise routine.

How will we know whether the plan is helping?

We agree a useful functional goal and review changes in symptoms and activity. The number of appointments depends on the findings and response. If progress is poor, the plan should be reassessed and another opinion or investigation considered where appropriate.
